These Terms & Conditions govern the use of the Janakalyan Health Card. By registering and using the card, you agree to the following:
1. Eligibility
Only individuals who provide complete and accurate details are eligible to receive and use the health card.
2. Usage Guidelines
- The card is valid only at listed partner hospitals.
- It must be presented to receive health services or benefits.
- The card is non-transferable.
3. Misuse Policy
Misuse or falsification of the card will lead to permanent disqualification and possible legal action.
4. Limitations
The card does not guarantee free treatment beyond the scope of Janakalyan provisions.
5. Updates
We reserve the right to change these terms at any time. Continued use of the card means you accept the updated terms.
